Documents required to begin financial planning process:

- Complete the personal information worksheet (required)
- Complete the net worth worksheet
- Complete the living expense worksheetÂ
- Complete the risk tolerance questionnaire
- Provide a summary of any life insurance policies in force
- Provide your most recent tax return
- Provide your most recent paystub(s)
- Provide a Total Rewards statement from your employer(s), if available
- Provide copies of your investment account statements, including current holdings
- Provide a list of investment options for any company-sponsored retirement plans (401(k), 403(b), etc.)
- Provide copies of statements for any debt you may have, including mortgage, car loans, credit cards, student loans, or home equity lines of credit
- If completing a retirement plan, provide your current Social Security statements, which can be accessed at www.ssa.gov
- If you would like us to review your current estate plan, provide copies of your trust, will, and powers of attorney.
